Substitute Application and Required Forms

Follow these steps to become a substitute in LISD: A. Prepare your Sub Application and Forms:
- Open Adobe documents (listed below)
- Type your information on the Adobe form
- Print the Adobe form (if you desire a copy for your files, print two. The forms cannot be saved electronically on many systems)
- Sign the forms where indicated IN BLUE INK.
- Hold your application and forms until the Paperwork Submission Session (Step "B")
APPLICATION AND FORMS (Both Item #1 and Item #2 ARE REQUIRED)
B. Attend a Paperwork Submission Session:
NOTE: Applications and forms are ONLY accepted at the Paperwork Submission Session! Click here for schedule. Paperwork submission session is handled on a first come-first served basis. We will continue to accept applications until the listed training session is full. Once we have reached capacity, you will need to attend the next Paperwork Submission Session.
The following is REQUIRED for admittance into the Paperwork Submission Session - NO EXCEPTIONS!
- Original signed application and forms prepared in section "A".
- Bring ID with you as specified in the I-9 Form, last page
- Proof of education - PHOTO COPIES of College transcripts showing 48 hours, Degree and/or Proof of Teacher Certification
- Your best smile! Your photo will be taken for your ID Badge.
C. Attend our MANDATORY Training Session:
- You will be registered for your training session after submitting your application and forms at the Paperwork Submission Session.
- You are required to download and bring a copy of the LISD Substitute Handbook in order to be Admitted to the training session. Click Here for Handbook (pdf).
- Please note the following: Location is different for Mandatory Training Session; childcare is NOT available; dress in business casual. The training session will take 1/2 or a full day, depending on your experience. Click here for details
