Please arrive at least 15 minutes early to have your picture made for your identification badge.

Instructions for Returning Paperwork: New Employees will need to bring the forms listed below, under "All New Employees" AND "Employment Category".

  • Fill out the form electronically
  • Print the form (If you can't save the form to your computer, print two forms if you desire a copy for your files)
  • Sign the form where indicated and bring to your Orientation Meeting

All New Employees: Please bring the following forms to your new hire meeting:

Professional Employees

Items of Interest
to New LISD Employees


  • Official Transcripts (teachers and administrators only): your personnel file will remain incomplete until official, hard copy of transcripts are received. Transcripts from ALL colleges and universities attended are required. You may have to contact your school and arrange to have the official transcripts sent to us. (Melinda Sullivan, Human Resource Services, Lewisville ISD, P. O. Box 217, Lewisville TX 75067)
  • Proof of Education (for all other positions): Official transcripts of college hours, if applicable, or copy of high school diploma, or GED.
  • Service Records: For experience to be calcuated correctly, original service records must be received; otherwise, salary will be calculated at entry level experience.(a copy of your service records is acceptable to place you on the pay scale until the originals are received.) In the case of multiple service records, when ALL service records are received in HRS, one payroll change will be issued.
  • First Year Teacher Support Program: LISD provides assistance and support to First Year Teachers (FYT) with the First Year Teacher Program. Teachers who are new to the profession (first year) qualify to participate. A Statement of Understanding is required for all First Year Teachers.
  • Your best smile! Your picture will be taken for your Employee ID badge.
